To make a return appointment, or cancel/reschedule an existing appointment, please call the division directly. You can also useMyChart to manage your appointment for select specialties.
New Appointments: If this is a new appointment, consent forms/documentation must be signed at the visit by a legal guardian.
Return Appointments: If this is a return appointment, the consent forms/documentation will be expired. In order to prevent any interruptions in your child's care, please carefully read, sign and bring the four (4) documents shown below at the time of your child's appointment. These documents can be brought by the adult accompanying your child to the appointment, as long as they are signed by a legal guardian in advance. The documents will be scanned to your child's medical record and will be valid for one (1) year.

Each child will need their own set of signed consent forms/documents.
If these documents are not received at the time of the appointment, we may need to reschedule your child's appointment.
